An elevated PSA does not automatically mean cancer. It can rise due to benign prostatic hyperplasia (BPH), inflammation/infection, or other causes. The usual approach is to repeat the PSA test, perform a rectal exam, and, depending on the case, order imaging and evaluate whether a biopsy is needed.
